READ ALL OF MY SCREENING REQUIREMENTS BEFORE FILLING OUT MY FORM. I WILL NOT RESPOND TO FORMS WITHOUT ALL REQUIRED INFO. 

Please complete my screening form when you're ready.

I accept the following for screening:

1. A p411 account (my favorite).  I do still require an ID so I can make sure the account is yours. 

2. 2 or more whitelistings on TER.  Please send me screenshots of these.

4. Provide me with a references from other providers.  Give me the names of and contact information for models you've seen before.  They must have 20 reviews or more on TER or 10 okays or more on p411. ( I am very picky on models I accept references from. I must feel comfortable when i look at their profile they are reputable).

If you don't have a reference, you'll need to:

-Send a copy of your ID. Make sure it shows your full name, city, age and picture

-Send a selfie with todays date and a smile face written on a piece of paper

-Employment verification- this should include your name and the company you work for name on it. Work badge, check stub. For self employed a website, business card with a website that shows you own the company, an established linked in account ( please include link). 

-I will only be able to see you at my in call location.

If after doing your screening I am not feeling comfortable I may ask for a deposit as well. If you have canceled an appointment with out proper time notice I will ask you to start giving me a deposit to see me again. 

If at anytime if I start feeling uncomfortable with conversation or even our meetings I will end communication completely with you without explanation. Please remember this is not a personal relationship. Respect my private life and do not overstep boundaries as me being your provider.
